What Will You Do in This Role?
As a Lotus Nurse | Care Coordinator (RN/LPN), you are responsible for the training and education of the team members that provide care and services to the Lotus Guests (rapid recovery and rehabilitation), as well as the care management of Lotus Guests.

RN/LPN
Must have intermediate to advanced computer skills, including experience working with an EMR or EHR.
Must be able to read, write, speak, and understand the English language.
Must have outstanding verbal and written communication skills.
Possess the ability to make independent decisions when circumstances warrant such action.
Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle team members based on whatever maturity level at which they are currently functioning.
Must possess leadership ability and the willingness to work harmoniously with members of the team.
Must be organized and demonstrate time management skills.
Must maintain the care and use of administrative supplies, equipment, etc., and maintain the assigned office in a clean, safe and attractive manner.
Must function independently, have flexibility, personal integrity, and the ability to work effectively with team members.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Duties and Responsibilities | |
ADMISSION PROCESS | |
Collaborate with Admissions Coordinator to ensure room readiness for new admissions with appropriate equipment | |
Greet new guests in lobby in collaboration with admissions coordinator, orient to room, and explain Lotus | |
Make contact with resident representative, introduce self and update on admission | |
Discuss with guest and resident representative customer service survey process and Google reviews in collaboration with admissions coordinator. | |
Provide guests and resident representatives with contact information for community and for Lotus Care Coordinator | |
Collaborate with charge nurse and clinical management team to ensure admission assessment and orders are completed | |
DISCHARGE PROCESS | |
Review Recapitulation of Stay UDA for those that have plans to discharge and coordinate discharge plans | |
Complete discharge summaries with guests and responsible party as applicable prior to discharge | |
Review discharge medications, call changed medications into home pharmacy with a 30-day supply. | |
Post discharge call at 48 hours | |
DAILY RESPONSIBILITIES | |
Meet with DON and clinical team daily. | |
Check in with charge nurses for any changes, updates, and staffing | |
Round on all Lotus Guests | |
Review with charge nurse any residents experiencing a change of condition. | |
Review new admission medications with family and lotus guest. | |
Collaborate with ADONs and SS to ensure consults are ordered as applicable (Cardiology, nephrology, RD, RT, psych, Pulmonology, Physiatry) | |
Complete 48 hour meetings as applicable | |
Complete Unplanned Discharge Investigations as applicable | |
WEEKLY RESPONSIBILITIES | |
Attend care conferences | |
MONTHLY | |
Participate in QAPI | |
Lotus Care Coordinator call | |
MISCELLANEOUS | |
Support and participate in the Vision, Mission, and Values of Tutera and YOUNITE philosophy. | |
Comply with personnel, safety, and corporate policies and procedures. | |
Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator/DON or Nurse Manager on Call. | |
Treat residents, family members, visitors, and team members with respect and dignity. | |
Timely completion of assigned in-service training assignments. | |
Maintain the confidentiality of all resident and family information. | |
Report to work on time and as scheduled. |
