What Will You Do in This Role?
- Coordinate, supervise, and implement the Restorative Program to meet individual client needs.
- Collaborate with pharmacists, dietitians, physicians, staff, and patients to ensure quality care, efficient workflow, and regulatory compliance.
- Partner with management and staff to provide clinical coverage, training, and quality improvement initiatives.
- Ensure compliance with CMS, Tutera, and facility policies through management of patient intake, consents, and care plans.
- Review prescriptions and orders with the Doctor of Pharmacy to ensure accuracy and patient safety.
- Conduct routine audits to identify and correct medication entry errors.
- Address nutritional concerns by collaborating with the Registered Dietitian, monitoring weights, documenting recommendations, and updating care plans.
- Initiate medications, treatments, and patient orders in compliance with CMS, Tutera, and facility policies.
- Process new and re-admissions by entering medications, treatments, and other orders; initiated batch orders per facility policy and maintained care plans.
- Obtain consents for enablers and psychotropic medications
- Review daily order listings to verify accuracy and compliance, activated batch orders, and updated care plans accordingly.
- Monitor PRN medications to ensure appropriate use and prevent regulatory citations.
- Monitor dashboard for RA and CNA alerts and took appropriate action.
- Monitored rehab unit dashboard for new antibiotic orders, updating care plans and isolation/enhanced precautions as needed.
- Review high priority notes and ensure proper protocol is initiated and completed
- Review ADT statuses to ensure completion of discharges and admissions
- Post educational information to communication board and via 100 hall group messaging system
- Confirm orders ensuring compliance and accuracy. Update care plans as needed for new orders. Ensure Lab/DX orders are ordered and followed up for completion
- Support risk management by reviewing nurse input, finalizing reports with appropriate interventions, and updating care plans and communication boards.
- Monitored weights and vital signs, corrected outliers, and implemented appropriate interventions (e.g., consulting Registered Dietitian for new orders or re-weighs).
- Monitored UDAs on 100 Hall for completion and educated nursing staff to ensure accuracy and compliance.
- Oversaw and maintained the restorative nursing therapy program for Long-Term Care, ensuring timely completion of quarterly and PRN assessments; tailored programs to individual needs and coordinated with therapy to implement functional maintenance plans upon discharge.
- Coordinated CNA and RA tasks for the Restorative Nursing Therapy program to ensure accurate documentation; supervised RA compliance, collaborated to address individual needs, maintained weekly program spreadsheets on the shared drive, and assigned RA duties as needed.
- Complete ISNP billing log monthly and forward to Business Office Manager on the first of every month
- Assist to maintain MCS board in office weekly for accuracy, consents, orders, and care plan updates. Perform room audits and order listings to ensure listings are complete and correct
- Assist with special projects, audits, QAPI, state tasks, in-services, education as assigned by the DON and Administrator
- Comply with personnel, safety, and corporate policies and procedures.
- Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator/DON or Nurse Manager on Call.
- Treat residents, family members, visitors, and team members with respect and dignity.
- Timely completion of assigned in-service training assignments.
- Maintain the confidentiality of all resident and family information.
- Report to work on time and as scheduled.
